Overview and Goal(s)


Placement of a catheter in alternate vessels including the saphenous, cephalic, and lateral thoracic veins



Indications


For administration of intravenous fluids and medications when jugular catheterization is not possible



Contraindications




• Cellulitis in surrounding tissues


• Phlebitis of the vein to be catheterized


• Saphenous veins are not safely accessible in most adults



Equipment, Anesthesia




• Clippers


• Sterile prep


• Exam gloves for scrub preparation


• Sterile gloves


• 2- to 3-mL lidocaine with a 25-gauge needle attached


• Catheter/extension set/catheter injection cap


• Heparinized saline


• Suture (2-0, polypropylene)


• White tape


• Scissors


• Bandage material depending on placement



Anticipated Time


Approximately 15 minutes, including scrub



Preparation: Important Checkpoints




• If catheterizing the lateral thoracic vein, ultrasound-guided identification of the cranial and caudal direction the vein is coursing under the skin assists in catheterization.


image Landmarks can be marked with a permanent marker on the skin outside the prepped area.


Possible Complications and Common Errors to be Avoided


Avoid large volumes of subcutaneous block, which can compress the vein or shift it deeper.



Procedure




• Restrain the horse with sedation, or a twitch.


image For foals, restrain with light sedation in lateral recumbency.
