Definition


Physical obstruction of the small colon caused by external compression when it becomes entwined with a pedunculated lipoma



Epidemiology



Species, Age, Sex




• This condition is rarely seen in horses younger than 9 years; it is mainly seen in horses older than 15 years.


• Geldings appear more predisposed than mares or stallions.



Genetics and Breed Predisposition


Ponies, Quarter Horses, and Morgans have been reported to be at an increased risk.



Risk Factors


Fat horses



Clinical Presentation



History, Chief Complaint




• Colic signs


• Decreased or absent fecal output


• Tympany, gas distension



Physical Exam Findings




• Normothermic but hyperthermic or hypothermic if severely compromised


• Often tachycardic


• Sometimes depressed and lethargic


• Mucous membranes are variable; most commonly, they are injected or toxic and tacky


• Abdominal distension



Etiology and Pathophysiology


Mesenteric lipoma normally on a stalk wraps around segment of small colon



• Extraluminal obstruction


• Strangulation of mesenteric vessels

• Small colon enterolith


• Small colon fecalith


• Small colon bezoar


• Small colon impaction


• Small colon phytoconglobate


• Small colon foreign body


• Small colon intramural hematoma


• Small colon neoplasia


• Small colon intussusception


• Small colon volvulus



Initial Database




• Complete blood count: Leukopenia or leukocytosis common; occasionally normal


• Rectal examination


image Possibly difficult to enter abdomen because of straining or physical obstruction of small colon
