Definition


Hemoglobinuria is the presence of hemoglobin in the urine; it is associated with red to amber colored transparent urine that remains pigmented after centrifugation.

Physiology


Free hemoglobin in the plasma splits into dimers that are immediately bound to haptoglobin and cleared by hepatocytes by the formation of bilirubin, iron, and amino acids or bound to macrophages. If haptoglobin is saturated, hemoglobin dimers accumulate in the plasma (hemoglobinemia) and pass through the glomerulus where they are resorbed and degraded by proximal tubule cells or excreted in the urine (hemoglobinuria). Hemoglobinuria will cause a positive blood test on a urine dipstick.



Causes of Abnormally High Levels




• Severe intravascular hemolysis


• Hemoglobinuria may also be present in cases of hematuria in which red blood cells have lysed (as with dilute or alkaline samples or delayed processing); however, hemoglobinemia should be absent.
